I want to fix an error in my submission, but my form is locked!

When an authorized user submits your form, it is locked and cannot be edited. This is done to prevent multiple users from submitting different versions of your facility's application, a condition guaranteed to cause confusion all around. If your form has been submitted and you discover an error which you wish to correct, please write to the Help Desk giving us your facility name and your user name. We can unlock the form for you.

Do I have to re-enter all my data from last year?

If you submitted a SAFE application last year using our secure online form, then your old data will automatically appear in the appropriate places on this year's form.

I can't log in!

We've rounded up the usual suspects:

  1. Passwords are cAsE-sEnSiTiVe, so make sure that you haven't entered a character in the wrong case.
  2. So are uSeR NaMeS.
  3. Verify that you are using this year's user name and password. You were sent an activation link which, when clicked, sent you an e-mail with your current year account details.
  4. Make sure you are trying to log in to the correct login page. The link to this year's log in page is found on the SAFE Program page.
  5. You are trying to use the activation code instead of a real password. The activation code is not a password. It only works (one time) to activate your account.
  6. If you never got, or have lost, the e-mail containing your activation link, then contact the person who submitted your facility's application last year. They are (usually) your facility's master user and have the power to create an account for you or to look up your log-on details. If you can't figure out who your facility's master user is, then e-mail us and we'll be glad to help.

If none of these suggestions seem to fix the problem, then send us an e-mail. We'll review the log files to discover the problem.

I've activated the account, but...

Now and then the legitimate owner of an account forwards his account creation notice to someone else, thinking that he has passed on the power to use the account. He hasn't! He's just created a lot of fuss and trouble for everyone else.

Should you be forwarded an activation link and you activate that person's account, the password and login details are mailed to the real owner of the account...not you. Accounts cannot be "transferred." If you need an account, ask your facility's master user to create one for you. If you can't figure out who your master user is, then write to the LCA Help Desk.

Will my plant win?

Looking into our crystal ball, we see . . . hmn.

It appears that there is no way for us to know until you have sent us your application. However, we can promise that everyone who fails to apply will win absolutely nothing: no award, no recognition, no congratulations, and no report on how their facility compared with others.

I only have one user account. Can my facility have two (or more)?

Of course! Unless something weird has happened, your single user account should be a "master" account. That means you can use it to make lots of little accounts all by yourself. You saved the e-mail that was sent to you when you activated your account. (Yes, you did. You were told to save that e-mail right there in its very first sentence.) Just call up that e-mail and click on the magic link to reach the master user control panel. After that, just follow directions.

Can we have more than one master account?

The short answer is "It's not a good idea."

Since the master user can do things like change passwords, create accounts, delete accounts, broadcast e-mails to all of your team's users, and submit the form, etc., you really, really don't want two people doing that at once unless they are Chang and Eng Bunker (the original "Siamese twins").

Some facilities use a team which works cooperatively to assemble data for their application. You may create user accounts which can

  1. view one or more sections of the form, but not change anything;
  2. view and change answers for one or more sections, but not submit the final application;
  3. or
  4. view and edit anything and submit the application.

(We recommend that you limit the number of users at your facility with the authority to submit the form. Submission is "final" and the form cannot be changed after that, even by the master user.)

Now that you've read the long answer, the answer is "Yes, if you promise us that your company's Siamese twins will get along really well."

I'm trying to edit my application data and it won't let me!

Oh-oh. You or someone else at your facility submitted your form, instead of saving it. True, both words begin with "S," but beyond that they have no similarity. Pressing "Submit" (which you have to do in two places to make it actually happen) means you are sending your application to us for consideration. From that point on, your form is locked against further changes. "Save" means you just want to turn off your computer so you can finally go home at six o'clock, or seven, or whenever.

If your form has been submitted and locked, but you still need to make changes, please write to the Help Desk. Tell us your facility name, your user name and, if you say "pretty-please," we will unlock your form for you.

Is my online data secure?

We have made every effort to protect the security of your data. Our bounce-back activation system forces every user account to be associated with a valid e-mail address. All of your communications with login screens and data forms occur over the same kind of encrypted links used to take credit card data and your data files are maintained on a locked-down server. All comings and goings are logged and we review the log activity daily.

What is Severity Rate (SR) and how is it calculated?

The Severity Rate is calculated by summing all days affected by injury, then multiplying the result by the theoretical hours that 100 employees would have worked divided by the hours that your employees actually worked. The result is a standardized metric which might loosely be described as "days lost to injury per 100 employees."

The SR is calculated with this formula:

R = days of restricted activity
W = total hours worked

The number 200,000 used above is the theoretical number of hours that 100 employees would work at 40 hours per week over a year of fifty weeks. All of the required data is found on your facility's OSHA log.
